Ah, that's the only clue I need (overhead and flat bench aggravate it). Much of this is due to the fact the wrist, shoulder and particularly elbow are lined up in an un-natural fashion. If you ask someone (anyone) to pick a baby up overhead, they push up and AWAY, keeping the elbow in line with the body. Likewise, if you trip and fall forward your body instinctively assumes its strongest anatomical position: Your hand will be in line with your elbows, not splayed out at 90 degrees as seen in the bench/overhead press. 

YOUR SOLUTION: The barbell jammer press!

Give that a whirl Mike, it's a fantastic movement. Works all 3 heads of the shoulder and corrects unilateral imbalances. I personally perform these on one knee, with my other arm behind my back. This, to take the "body english" out of the movement. When working the right arm, kneel on the right knee. Same prescription when you switch arms for the left.

I have guys with blown rotator cuffs that can no longer bench/overhead press. Using this exercise though, they can train hard, heavy and pain free! :)
